Troop forms and applications

Whether you are the lead for a troop meeting, a campout, or event, we have a step-by-step guide or template you can follow to ensure it’s a success.

 

Medical Form

This form must be renewed each year to attend summer camp.   The same form is used for Scouts and adults. 

 

Permission Slip

This form must be signed by a parent of each Scout when going on an outing.   The event coordinator may wish to edit the form with pertinent information and then send to all the attendees to sign.

 

Expenditure Approval Form

If you plan to make a purchase for the troop, this form needs to be submitted to the Treasured PRIOR to the expense being incurred.

 

Annual Troop Dues Form

In September of each year, each family fills this out to show their intention to continue their son’s membership for the year and returns this form to the Treasurer. 

 

New Scout Fees Form

When a new scout joins the troop, this form lists the fees for starting his membership.

 

Scout Advancement Record

All Scouts should use this form to keep an ongoing record of all of their participation and achievements.  This form is reviewed during his Board of Review and will come in handy as he prepares for his Eagle rank!

 

Driver Form

All adults who wish to drive Scouts to events need to provide the information in this form to the person who maintains the transportation records (See Adult Leadership).

 

Volunteer Form

The troop’s success depends on willing adults.   Submit this form to let the Scoutmaster and Committee Chairman know where you would like to participate.

 

Troop Scholarship Application

If lack of personal funds for camp or dues would prevent your Scout from participating, please apply for a troop scholarship.   We want to keep your Scout active!

 

Scout Application

New Scouts should contact the New Parent Coordinator for this form.

 

Adult Volunteer Application

Adults who wish to participate in the troop as an adult leader, event coordinator or merit badge counselor should contact the New Parent Coordinator for this form.

 

Merit Badge Counselor form

Adults who wish to be a merit badge counselor should contact the Advancement Chairman for this form.  This form lists the merit badges that you can select from.
